A moderately sized town that was once called Red Mound was establishedback in 1828, when its first settlers arrived in the area. Red Mound would later be renamed after Benjamin Rush Milam, a valiant soldier in the Texas Revolution. Historical documents show that Milam was incorporated in 1837, making it the very first incorporated town in East Texas.

It was originally the municipal seat of Sabine County until 1858. Later on, its incorporation was dissolved and today it is a census-designated place (CDP). Situated on the renowned El Camino Real, Milan can be found seven miles north of Hemphill at the intersection of State Highway 87 and State Highway 21.
